Why must I submit a completed health form to the Director of Health Services prior to entrance to Converse College? 

 

This information is essential to the staff when administering medical care.  The medical history enables the staff to assist the student in the prevention of future health problems.  This provides documentation of your physical condition, immunization status, exposure to Tuberculosis, Allergies to medications or other environmental agents, medications presently taking and special problems.

 

Why does Health Services need to know about the medications I take?

 

From the time you arrive at Converse, your health needs are our concern.  To establish these needs,  we must know or recognize them.  This is why we stress the importance of the medical record as a baseline of health information.  This record will tell us what is normal for you and what problems you have had and have now in order to determine your future needs.  You must register any medicines that you bring to campus with Health Services and inform the Nurse Practitioner whenever you are treated.  All of your records are kept strictly confidential.

 

Do I have to have all those shots and the whole medical form completed?

 

Yes.  State Law requires this documentation be complete.  The only exception is religious exemption for immunizations which requires documentation from your Physician or local health department.  The Tuberculin Skin Test (PPD) is required, because this is a medical test determining your exposure to Tuberculosis.

 

What if I need to see the doctor when he/she is not at Health Services?

 

You must first be evaluated by the Registered Nurse in Health Services, who will then schedule an appointment with a physician.

 

What if I don't have transportation to the doctor's office?

 

If a friend or roommate is not available, Health Services has made arrangements with Spartanburg Regional Medical Center Transport Services. Health Services must call to make arrangements for this service. The fee is $3.00 for a one way trip.

 

What if I should become sick during the night? 

 

You should contact your C.A. (community advisor) or R.C. (resident counselor).  These people live in your dorms and are equipped to help you with your illness.  If you have an emergency call Campus Safety at Ext. 9026. In addition, you may leave a message on the registered nurse's voice mail, Ext. 9116, and your call will be returned when Health Services opens the next morning.  

 

What if I am sick on the weekend? 

 

If you become ill during the weekend, contact the C.A. or R.C. on call.  They will help you determine what you should do.

 

What are C.A.s and R.C.s and what kind of health care training do they have?

 

A Community Advisor (C.A.) is a student, who resides on the same floor in your dorm, and is an advisor for all the students on that floor. There is one Resident Counselor (R.C.), who is normally a graduate student, for each dorm.  At the beginning of each school year, the Director of Health Services certifies each C.A. and R.C. in American Red Cross CPR and trains them in first aid.

 

Can my parents call Health Services to find out why I was there?

 

No. Information from health treatment is confidential.  Health Services goes to great lengths to provide a secure environment for your medical records. The only way anyone can get any information from your health records is with your written or verbal permission.

 

Can Health Services send the health services bills home to my parents?

 

Yes, with your verbal permission.
