FORMS FOR INCOMING FRESHMEN AND TRANSFER STUDENTS

Below you will find a description of forms that will need to be completed as an incoming freshmen or transfer student. As these forms are updated for the incoming freshmen class, they will be posted on this page for download. They will also be mailed to your residence or given to you when you arrive on campus.

Letter from the Dean of Students
This is a welcome letter that reviews forms that will need to be submitted. It also explains what an incoming freshmen or transfer student needs to do if she plans to live on campus. Finally, this letter gives information about forms and information the student  will receive through mail in the coming months. 

Student Information Form
(Mailed to student in March)
This is a biographical information form that is required for all students by Converse College and is kept in the Dean of Student's Office.

Housing Agreement
(Mailed to student in March)
This form must be read and signed by incoming freshmen and transfer students who intend on living on campus in order to be placed in a room.  Please complete and return this form as soon as possible.  

Health Form
(Mailed to student in March)
This form is required to be completed and returned as soon as possible to Health Services. Students will not be allowed to register unless this form has been returned.

Summer Programs brochure and registration
(Posted and mailed to student in May)
This brochure includes information regarding opportunities for community service for incoming freshmen and transfer students. Visit the Summer Programs website.

Academic Advising Forms
(Mailed to student in May)
A packet will be sent to incoming freshmen and transfer students in May. It will explain Converse Academic Advising and will include forms with directions for the student to submit.

New Student Preview
(Posted and mailed to student in July)
This is an informational news letter for incoming freshmen and transfer students.  It includes a letter from the President, important dates, residence hall information, opportunities for involvement, information about the Big Sister/ Little Sister program, important campus telephone numbers, etc...

Parent Letter
(Posted and mailed to student in July)
This is an informational letter for parents of incoming freshmen and transfers that gives information about dates, new student check-in, and parent orientation.

Information Technology Services Brochure
(Posted and mailed to student in July)

This brochure gives information about technology at Converse, requirements and recommendations for personal computers, and frequently asked questions.

Directory Exclusion Form
(Posted and mailed to student in July)
A new student will complete and return this form to the The Division of Student Affairs Office if she does not want personal information included in the campus phone directory.

Dining Services Brochure
(Posted and mailed to student in July)
This includes information on meal plans, the Gee Dining Hall hours and services, and Sneakers Coffee Shop hours and services.

Loft Policy for Residence Halls
Students may design their lofts provided they meet the following guidelines.

Patient Insurance Information Form
(Mailed to student in June)
The parent of each student-athlete is required to complete a Patient Insurance Information Form. Our Athletic Trainer must have this completed form before an athlete can begin practice or compete.

Student-Athlete Sports Information Form
(Posted and mailed to student in July)
Each student-athlete is asked to complete a Sports Information Form for our athletic web page. This will enable us to submit press releases for our athletes in a timely manner.
